There is an update to the 2016 UK Professional Standards for the Reporting, Learning, Sharing, Taking Action and Review of Incidents (also known as the Error Reporting Professional Standards). They are now known as the 'Patient Safety Professional Standards - Responding to Patient Safety Incidents', by the RPS, Association of Pharmacy Technicians UK and Pharmacy Forum Northern Ireland. The standards have now been published here.
